**Check-ups for NHS patients**

If you were an 'active'  NHS patient with the practice when Covid-19 occurred and we have not yet seen you for an examination please can you contact us so we can make sure you are on the list for an exam appointment.

If you have had an exam appointment since Covid-19
We hope to be able to see you for a repeat exam as soon as we can; your recall interval may be extended at the moment - this is because we need to try and see all of our NHS patients. If you are a child, have diagnosed periodontal disease or a medical condition which impacts upon your oral health we will be doing our very best to keep you at or as close as we can to the required exam interval.  We will be doing our very best to see all urgent dental conditions.  Please ask if you need help or further explanation re the above.
Orthodontic Treatment
If you have a child who you think may need orthodontic treatment please can you contact us so we can assess if they are eligible for NHS referral.

Your Forms

We may ask if you will complete your 'forms' (medical history) in advance of your appointment by filling them out from a link.  Please make sure we have your up-to-date email address. Thank you for your help doing this.
